Upon contacting Fingerprints Therapy Services, we will collect general information about your child and insurance information.
Prior to your child's evaluation we will need a script from your child's physician.
You will be assigned to a therapist who best matches your child’s needs and availability. Your first visit will then be scheduled.
We kindly ask that if you are unable to make the initial evaluation, please call to cancel 24 hours prior and we will reschedule for a different time.
The initial evaluation and following sessions will last approximately 55 minutes.
